Your First Appointment
Please plan to arrive 30 minutes before your appointment time so we can complete
the necessary forms for your health record.
Our provider will also ask many questions about your
medical history for future visits.
Bring all current medications with you. Health information is quite personal and we assure complete and utmost confidentiality
of your health records.

Payment Arrangements
SJSFHC accepts most private insurances, Medicare, and Medicaid
(including Absolute Total Care, Select Health, & Unison, & Carolina Medical Home Network).
If there is a co-pay or deductible for your plan, payment is expected at the time of service.

Payment Option
We accept Medicaid, Medicare and most private insurances.
Our centers also offer a sliding-fee
discount program for eligible low income families.
If you qualify for the sliding fee we ask that you bring a minimum of $25.00 to each visit thereafter.
We will work with you to establish a payment plan
to that meets your financial status.

Appointments
It is important that you schedule necessary follow-up appointments.
Failure to keep your scheduled appointments at SJSFHC hinders our ability
to provide the best care to our patients. We ask that you will please notify us less than 24 hours
ahead of time if you must cancel an appointment. This will allow us the opportunity to offer that appointment to another patient.
Walk-in appointments are available as the schedule allows.

Your Registration Checklist
Registration is required on or before your first visit.
(State license, ID, and Social Security Card)
(Private Insurance, Medicare, Medicaid, Other)
Proof of Address (Phone bill, Utility bill, etc.)
Proof of Income (If requesting discount)
Bring the following items to your first appointment:
Proof of unemployment; food stamps; housing; and/or child support
Social Security Income Statement
Previous year's income tax return
Any other source of household income
Once we have reviewed this information, we will assist you in filling out an application for financial assistance.
There is a $25.00 minimal fee for each visit, regardless of the discount given. If you do not have insurance, you may also
qualify for medication assistance.
