Please plan to arrive 30 minutes before your appointment time so we can complete the necessary forms for your health record.

Our provider will also ask many questions about your medical history for future visits.

Bring all current medications with you.

Payment Option


We accept Medicaid, Medicare and most private insurances.

Our centers also offer a sliding-fee discount program for eligible low income families.

If you qualify for the sliding fee we ask that you bring a minimum of $25.00 to each visit thereafter.

We will work with you to establish a payment plan to that meets your financial status.

Appointments


It is important that you schedule necessary follow-up appointments.

Failure to keep your scheduled appointments at SJSFHC hinders our ability to provide the best care to our patients.

We ask that you will please notify us less than 24 hours ahead of time if you must cancel an appointment.

This will allow us the opportunity to offer that appointment to another patient.

Walk-in appointments are available as the schedule allows.
